of professional education including qualification documents and
details of basic and post-basic education, as well as professional
experience. There must be references from the UK and the applicant's
Member State. Evidence is required of good conduct.

- Documentation of applicant's name, date of birth and nationality
(e.g. copies of relevant pages from passport).
