include evidence of physiotherapy education with diploma.
- Evidence of physiotherapy qualification (diploma)
- Evidence of good conduct
- Curriculum Vitae
- Documentation of applicant's name, date of birth and nationality
(e.g. copies of relevant pages from passport).
- Confirmation by the applicants
- Name of competent or other National Authority
