The application must be submitted in the form
of certified copies or original documents, and include:
- Evidence of qualification (with diploma)
- Certificate from the authorities confirming that the applicant
has the right to practise as a physiotherapist in his/her country.
- Evidence of good conduct
- Curriculum Vitae Documentation of applicant's name, date of
birth and nationality (e.g. copies of relevant pages from passport).
- Confirmation by the applicant
- Name of competent or other National Authority
